Enrolment of your child

If your child is a New Entrant

Complete the enrolment form and the following documents:

• Green Health form (found in the centre of the Healthy at Five booklet)
• Immunisation Certificate
• Copy of Birth Certificate or Passport
• Trip Consent form
• Asthma Action Plan if required
• Emergency Collection Form
• Website permission form

For all other enrolments

Complete the enrolment form and the following documents:

• Copy of Birth Certificate or Passport
• Trip Consent form
• Asthma Action Plan if required
• Emergency Collection Form
• Website permission form

Optional extras for both enrolments

• HASCAP enrolment form
• Telecom Sponsorship form
• Manchester Unity school banking account

NB:If you are not a resident of New Zealand and recently arrived in the country you will need to provide a copy of your passport and relevant paperwork for your entry permit.
